InsurancePlease present your insurance identification card at the time of your appointment. If we have a contract with your health insurance plan, we will file a claim for your services.

At the UAMS Family Medical Centers, we accept most major insurance plans including:

  • QualChoice
  • Blue Cross/ Blue Shield
  • UnitedHealthcare
  • Medicare
  • Medicaid

A number of smaller insurance companies are also accepted. We recommend that you verify your doctor is contracted with your plan.

Any co-payment, coinsurance and/or deductible are due at the time of service. Please be prepared to pay this amount.

  • A co-payment is normally a fixed dollar amount per office visit, identified on your insurance care.
  • Coinsurance is the percentage of the bill that is the patient’s responsibility.
  • A deductible is a fixed dollar amount that must be paid before the insurance will begin to pay.

If you do not have your current insurance identification card or other acceptable proof of insurance, your visit will be considered self-pay and you will be responsible for full payment at the time of service.

It is, at all times, your responsibility to follow up on all requests from your insurance company regarding claims and to question any unpaid insurance claims. If you do not receive an explanation of benefits (EOB) from your insurance plan within 60 days of your visit, please call that company. After payment or denial from your insurance, we will bill you for any remaining balance or noncovered services. The amount is due upon receipt of your statement, unless prior arrangements have been made with our financial counselor. Please remember that your insurance carrier makes the final determination regarding payment to our clinic.
